A Smooth Transition

Smooth Transition provides the guidelines and practice resources that govern how and when young people are offered Home Stretch WA, and the processes of referral, onboarding and working together with the Child Protection System.

Training and support is provided to each District Office and their Home Stretch WA provider through an Establishment Workshop. this helps to build a common understanding and a clear trusted pathway for young people to engage with Home Stretch.

Smooth Transition also is about supporting young people to smoothly transition into other professional supports by the time the reach 21. This might include support through a Leaving Care Program, Housing, Mental Health or other mainstream services.
