Practice Areas
The Home Stretch WA Model was built around a number of practice areas using particular practice approaches that were developed to be offered through a trained Home Stretch WA Transition Coach and Home Stretch WA Team. Each of these practice areas is interdependent, and together they are designed to be delivered in an integrated and seamless way.
Every young person will receive an individualised service based on their needs and aspirations, offered through a persistent, consistent, hope inducing relationship with their Transition Coach. This work must be underpinned by alignment with the Transition Coach Practice Principles, and delivered in a way that is consistent with the Home Stretch WA Model Integrity Framework.
Each practice area has its own guidelines, resources, tools and documents. As Home Stretch WA providers become skilled and confident in delivering services that are consistent with the Model, the Community of Practice will support service providers to refine and adapt the Model to meet the emerging needs of young people across the state.
