Home Stretch WA service is delivered with integrity to the model as co-designed with young people and Aboriginal Community. The policies and program management tools and resources support providers to ensure young people in out-of-home-care, who are approaching 18 years of age all over the state, can choose to access the Home Stretch WA service as it was intended.

Model Integrity Framework

The Home Stretch WA Model Integrity Framework is designed as a tool to support service providers to establish a high-quality Home Stretch WA service that is consistent with the service model co-designed by and with young people. It communicates a shared understanding of what the Home Stretch WA service must offer in WA to be considered a Home Stretch WA service. The framework also describes how Home Stretch WA should be delivered based on the evidence base and insights shared by members of the Home Stretch WA - Community of Practice.

The draft of the Model Integrity Framework has now been released for feedback and comment (click on the front page image below to access it or the table of documents at the bottom of the page) and will be on the Agenda for discussion at the next Guiding Panel Meeting in July 2023. All the tools and resources to operate and manage the Home Stretch WA program are found here.  Team position descriptions, onboarding checklists, FAQ, a copy of the Model diagram and guides for supporting staff are included below.

 

Over the next few months, the CoP will be adding a policy library for providers to access generic policies around service delivery, as well as other program infrastructure resources around supervision, case discussion and other workforce development support.
